Planning IOL Explantation or Exchange

If you are considering an IOL Explantation or Exchange for an IOL related problem which you feel is impacting on patient outcomes or safety, it is very important that you make the appropriate agencies aware of the situation, so that the adverse can be recorded and assessed.  These are the steps which are recommended:

1) Report the Explantation/Exchange to the appropriate agency in your country (See "Reporting" on home page).

2) Please report to the ASCRS Foldable IOL Survey, at the following link.

Fill out this form, and print out a copy of the report, for your files, before submitting.

3) The specimen should be placed in BSS (Balanced Salt Solution), as soon as possible. Include a copy of the "ASCRS Foldable IOL Survey" report that you printed, with the specimen.
